Job summary

Interim HealthCare in Madison, WI is seeking a Home Health Physical Therapist Assistant (PTA) to provide in-home therapy, help patients regain mobility, reduce pain, and achieve personalized goals. You will work with a home health team to execute care plans and monitor progress in patients' daily environments.

Qualifications

• Graduate of an accredited Physical Therapist Assistant Program with an active WI PTA license.
• Minimum of 1 year of physical therapy experience, ideally in home healthcare.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based physical therapy to patients who have limited mobility and function.
• Collaborate with an interprofessional home health team (RN, LPN, CNA, HHA, OT, PT, SLP) to implement the plan of care.
• Observe movements, document progress, and adjust the care plan as needed.
• Assist patients with exercises to increase mobility and manage pain.
• Educate patients and families on the plan of care, goals, exercises, and self-care.
• Coach families on safe home exercises and care tasks.
