Job summary

Trinity Health At Home is seeking a Home Care Physical Therapist (PT) to deliver patient-centered therapy in home settings. You will assess, plan, and implement PT interventions to improve mobility and independence, working with a multidisciplinary team.

The role emphasizes autonomous home-based care, progress tracking, and a strong emphasis on compassionate, relationship-driven service with minimal on-call requirements.

Qualifications

• Bachelor’s degree with progressive experience or a Master’s degree in Physical Therapy.
• Active Physical Therapist license issued in Pennsylvania.
• Working knowledge of home health Conditions of Participation preferred.
• Home healthcare experience is a plus.
• Current driver’s license and reliable transportation.

Responsibilities

• Provide physical therapy services in patient homes.
• Collaborate with physicians, nurses, social workers, and aides.
• Use advanced technology and documentation systems to monitor progress.

Education
