Atrinity Home Health, part of the Optum family of businesses, is seeking a Physical Therapist in Home Health to assess patients, develop treatment plans, and deliver interventions to improve mobility and well-being. You will evaluate progress and coordinate with the health care team to optimize outcomes.

Responsibilities include performing functional assessments, implementing therapy plans, documenting visits, and communicating with patients, families, and physicians.

Qualifications

• Current Physical Therapy licensure in state of practice.
• CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsabilités

• Evaluates the patient's functional status and PT needs as part of Plan of Care.
• Provides PT services within state scope and plan of care.
• Observes and reports patient responses and changes to supervisor/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and documents progress.
• Develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
