Job summary

PARS Therapy seeks a compassionate Physical Therapist (PT) to join our home health team in Liberty, TX. The PT will evaluate patients at home, develop individualized plans, and provide interventions to restore function and reduce pain.

Collaboration with a multidisciplinary team and education for patients are key parts of the role. The ideal candidate holds a TX license, has experience in home health, and demonstrates adaptability, evidence-based practice, and strong communication skills.

Qualifications

• Active Physical Therapist license in TX.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patients.
• Knowledge of evidence-based treatment techniques.
• Excellent communication skills.

Responsibilities

• Evaluate patients in a home health setting.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions including exercise, gait training, manual therapy, balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, sessions, and progress in digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care to empower patient independence.
