Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team in Linden, TX. This onsite role focuses on evaluating mobility, creating personalized treatment plans, and delivering evidence-based care in patients' homes.

The ideal candidate will hold an active TX PT license, have a graduate degree in Physical Therapy, and experience in home health or rehab settings, with strong communication and adaptability to diverse patients.

Qualifications

• Active Physical Therapist license in TX
• Professional liability insurance coverage for TX PT license
• Graduate degree in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Adapt care approaches to diverse patients
• Evidence-based treatment techniques knowledge
• Excellent communication skills and professional demeanor

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs and safety
• Monitor progress and adjust plans to achieve outcomes
• Document evaluations, treatment sessions, and progress notes digitally
• Collaborate with physicians, nurses, and therapists
• Conduct home safety assessments and recommend fall prevention measures
• Deliver compassionate care to empower patients toward independence
