PARS Therapy in Liberty, TX is seeking a compassionate Home Health Physical Therapist Assistant (PTA) to support patients throughout recovery. You will help restore mobility and independence by delivering personalized, in-home care under the supervision of a licensed Physical Therapist.

The role emphasizes collaboration with a multidisciplinary team, development of individualized care plans, and patient education to empower recovery and long-term well-being in a home-based setting.

Qualifications

• Active Texas PTA license.
• CPR certification required.
• Professional liability insurance coverage.
• Home health or rehabilitation experience preferred.
• Strong collaboration within a multidisciplinary team.
• Clinical PTA experience and patient education skills.

Responsibilities

• Deliver in-home physical therapy under the supervision of a licensed Physical Therapist.
• Identify patient goals and implement interventions to meet mobility needs.
• Provide evidence-based, cost-effective treatments to improve movement.
• Develop and carry out individualized care plans.
• Create wellness and fitness programs tailored to patients.
• Educate and motivate patients to participate in rehabilitation.
• Promote health by improving strength, flexibility, coordination and balance.
• Collaborate with other healthcare professionals for coordinated care.
• Monitor progress and communicate updates to the supervising therapist.
• Accurately document services and maintain patient records.
• Perform additional duties as assigned by the supervising therapist.
