Job summary

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to evaluate and treat patients in the home setting and provide skilled care under physician orders. The role emphasizes mobility, self-care, and participation in specialty programs to improve outcomes.

Qualifications

• Must possess a valid state driver’s license.
• Must have automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities to improve mobility and function.
• Help patients achieve independence in self-care and daily living activities.
• Administer skilled care per physician orders and established plan of care under the direction of the Branch Director.
• Participate in Specialty Programs such as Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, and Total Shoulder Replacement programs.
