Job summary

UnitedHealth Group, through Mederi Caretenders (LHC Group), seeks a Physical Therapist to perform home health assessments, develop and implement treatment plans, and monitor patient progress across Florida.

You will evaluate functional status, coordinate with the care team, educate patients and families, and ensure therapy follows state regulations. Requires current PT licensure and a valid driver's license; CPR certification is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification preferred (or ability to obtain within 90 days of hire).

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports patient response to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Function in diverse home situations regardless of patient background.
