Job summary

UnitedHealth Group is seeking a Physical Therapist for Home Health. You will perform assessments, develop treatment plans, and provide interventions to improve patient well-being in a home setting.

Responsibilities include evaluating functional status, planning care with physicians, and documenting progress. Licensure, CPR, and reliable transportation are required; strong commitment to patient-centered care is essential.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing physical therapy, per plan of care, in coordination with the health care team.
• Observes and reports the patient's response to treatment and any changes to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
