Job summary

UnitedHealth Group’s Caretenders/Home Health team seeks an licensed Physical Therapist to assess and plan patient care in a home health setting, focusing on mobility, endurance, pain, and overall well‑being.

You will develop and implement PT plans, deliver interventions to improve function, document progress, and collaborate with nurses, physicians, and families while following state guidelines and facility policy.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to reliable transportation.
• Ability to function in any home situation regardless of patient demographics.

Responsibilities

• Evaluates the patient's functional status and PT needs; assists in developing therapy plan.
• Provides services within scope of practice per plan of care and state laws.
• Observes and reports patient response to treatment to supervising clinician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes, goals, and frequency of care.
