Job summary

UnitedHealth Group, within the Patient Care Home Health team, seeks a Physical Therapist to evaluate home care needs and develop a plan of care. You will perform assessments, implement interventions, and monitor progress in patients' homes.

Responsibilities include identifying functional limitations, coordinating with the care team, and documenting visits and outcomes. Qualified candidates hold a current PT license in the state and may require CPR certification.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Develops the therapy plan of care in coordination with the physician.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports the patient's response to treatment and any changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers about the plan of care, functional limitations, and progress towards goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
