Job summary

At Home Healthcare, a part of LHC Group within the UnitedHealth Group family, seeks a Physical Therapist in Home Health to assess patient needs, determine a treatment plan, and implement interventions to improve mobility and function while coordinating with the health care team.

Required: current Physical Therapy licensure in the practicing state and a valid driver's license; CPR certification preferred within 90 days.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and reliable transportation.
• CPR certification preferred within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient's response to treatment.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care.
• Visits patients per Plan of Treatment and submits progress notes.
• Develops PT plan of care within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function well in any home situation regardless of demographics.
