Job summary

UnitedHealth Group seeks a dedicated Physical Therapist (HAC) to restore function and mobility, delivering skilled therapy under physician-directed plans. You will assess patients, plan care, and coordinate with an interdisciplinary team to improve quality of life, in a home health/hospital-at-home context.

The role emphasizes documentation accuracy, regulatory compliance, and collaboration with physicians to adapt plans as patients progress.

Qualifications

• Licensed Physical Therapist with GA license or eligible to practice.
• CAPTE-accredited PT program.
• Current CPR certification.
• 1+ year of PT experience; or residency/mentorship.
• Driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards
