Job summary

UnitedHealth Group's Idaho Home Health unit seeks a Physical Therapist to assess patients, develop treatment plans, and implement interventions to improve mobility and function in a home health setting.

You will collaborate with physicians, nurses, and family members to monitor progress, adapt plans, and ensure high-quality care consistent with state requirements and company policies.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports the patient's response to treatment and any changes to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patients, family, and caregivers regarding the plan of care, limitations, and progress toward goals.
• Visits patients according to Plan of Treatment, completes progress notes for each visit, and submits notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Demonstrates ability to function in diverse home settings.
