Job summary

Elite Home Health, part of LHC Group within the Optum family, seeks a Physical Therapist to assess and plan care for home health patients. You will implement interventions to improve well-being, monitor progress, and collaborate with the health care team to tailor the plan of care to each patient.

Responsibilities include evaluating functional status, developing therapy plans, and reporting outcomes while adhering to state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice and plan of care.
• Observes and reports patient responses to treatment.
• Treats patients to relieve pain and restore function.
• Informs patient and family about plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
