Job summary

Qualifications

• Current Physical Therapy licensure in state of Oregon.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Assists physician in developing the therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervisor/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient, family and care team on plan of care and progress.
• Completes progress notes after each visit and maintains state guidelines for PT plans.
