Job summary

UnitedHealth Group seeks a Physical Therapist for Home Health to assess patients, develop care plans, and collaborate with the care team to improve mobility and well-being.

The role requires current PT licensure in the state and a valid driver's license. CPR certification is preferred. This full-time position offers a comprehensive benefits package and competitive compensation based on productivity.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing the practice of physical therapy, in accordance with the plan of care, and in coordination with other health care team members.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs patient, family, and care team regarding plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Maintain ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
