Job summary

Caretenders, a part of LHC Group, is seeking a Physical Therapist for Home Health in a per diem role with at least 30 hours per month. You will assess patients’ functional status, develop a PT plan of care, and provide in-home interventions to improve mobility and well-being.

You must hold current Physical Therapy licensure in the state, have a valid driver’s license and reliable transportation; CPR certification is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Assists the physician in developing the therapy plan of care.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care and progress.
• Completes progress notes for each visit and reports outcomes.
