Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs; assists in development of plan of care.
• Provides PT services within scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain and restore function while maintaining standards of practice.
• Instructs patients and caregivers about plan of care, limitations, and progress toward goals.
• Completes visits with progress notes and submits as required by policy.
• Develops PT plan of care within state guidelines; reports outcomes and frequency of care.
