Job summary

Ascension at Home, together with Compassus, seeks a licensed Home Health Physical Therapist in Pensacola, FL. You will plan, implement, and evaluate patient care plans to restore or maintain well-being, collaborating with an interdisciplinary team to provide coordinated home-based care.

The role requires a BS in Physical Therapy, current PT license, CPR certification, and 2+ years of relevant experience in acute or rehab settings. A commitment to infection control and quality care is essential.

Qualifications

• Bachelor of Science in Physical Therapy required.
• Two (2) years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred.
• Active and unencumbered Physical Therapist license in state(s) of employment required.
• Current CPR certification required.

Responsibilities

• Provides care utilizing infection control measures per OSHA.
• Accepts clinical assignments consistent with education and competence.
• Monitors cases to ensure compliance with third party payors.
• Communicates effectively with patient/family and team members to ensure coordination of services.
• Performs initial and ongoing clinical assessments including OASIS as required.
• Prepares and revises a Plan of Care in consultation with physician and care-team members.
• Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders.
• Instructs patient/family in transfer techniques and use of equipment.
• Performs other duties as assigned.
