Job summary

Ascension at Home together with Compassus is seeking a Home Health Physical Therapist to deliver one-on-one care in patients' homes, helping them regain strength and independence for daily living activities such as church or garden work.

Qualified candidates hold a BS in Physical Therapy, an active PT license, a valid driver’s license, CPR and TB/Hepatitis clearance, plus at least two years of PT experience in acute or rehab settings.

Qualifications

• Bachelor of Science in Physical Therapy.
• Current State License as a Physical Therapist.
• Valid Driver’s License.
• Current CPR, negative TB screen and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting.

Responsibilities

• Provide home health physical therapy to patients in their homes.
• Assess patients’ mobility, strength, and functional goals.
• Develop and implement rehabilitation plans.
• Coordinate care with families and other providers.
