Ascension at Home together with Compassus seeks a licensed Home Health Physical Therapist to assess and treat patients in home settings. You will plan, implement, and evaluate care plans to restore or maintain patient well-being while collaborating with an interdisciplinary team.

The role requires a BS in Physical Therapy, a valid state license, and CPR certification, with 2 years of acute care or rehab experience preferred. Excellent communication with patients and families is essential.

Formación

• Bachelor of Science in Physical Therapy required.
• Active PT license in the state of employment.
• CPR certification required.
• 2 years of PT experience in acute care or rehabilitation preferred.

Responsabilidades

• Provides care utilizing infection control measures.
• Participates in in-service programs and maintains competency.
• Performs initial and ongoing clinical assessments including OASIS when appropriate.
• Develops and revises Plan of Care in consultation with physician and care-team.
• Documents progress notes, orders, and other clinical documentation.
• Communicates with patient, family, and health-care team to coordinate services.
• Trains patient/family on transfer techniques and use of equipment.
• Monitors cases to ensure payor requirements are met.
