Job summary

Ascension at Home together with Compassus seeks a licensed Home Health Physical Therapist to plan, implement, and evaluate patient care plans in collaboration with the interdisciplinary team.

The role emphasizes infection control, accurate documentation, and educating patients about transfers and equipment. A BS in PT and current license plus CPR certification are required, with 2 years of acute care or rehab experience preferred.

Qualifications

• BS in Physical Therapy required.
• Two years experience in acute care or rehabilitation preferred.
• Active Physical Therapist license in state of employment.
• Current CPR certification required.

Responsibilities

• Provides care following infection control measures for patient and staff.
• Plans, implements, and evaluates patient care plans with the IDT.
• Performs initial and ongoing clinical assessments, including OASIS as appropriate.
• Documents progress notes, orders, and plan of care revisions.
• Educates patient/family on transfer techniques and device use as needed.
• Treats neuromuscular, neurological, and orthopedic disorders per referral.
