Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full-time opportunity in Baltimore, Maryland. The role focuses on delivering skilled home health visits to adult and geriatric clients in the Baltimore City area.

One year of licensed PT experience is required. You will assess, treat, and revise care plans, educate clients and families, and document all care using a state-of-the-art EMR as part of a multidisciplinary team dedicated to enabling patients to remain at home.

Qualifications

• Current MD Physical Therapist license.
• Graduation from an APTA/CAAHEA-approved PT program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, balance, strength, ambulation).
• Develop and implement individualized care plans including manual therapy and gait training.
• Reassess and revise the PT care plan with the nursing team as needed.
• Educate clients and families on rehabilitative care and activities for independence.
• Document observations and interventions in the EMR.
