Qualifications

• Current North Carolina PT license.
• Graduation from an APTA/CAAHEA-accredited program.
• Ability to work independently with strong time management.
• Excellent interpersonal, organizational, and problem-solving skills.
• Experience with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, gait, and safety assessments.
• Develop and implement individualized PT care plans with interventions.
• Collaborate with nursing and multidisciplinary team in care planning.
• Educate clients and families on rehabilitative care and safety at home.
• Document observations and interventions using a touch-pad tablet.
• Apply wound care interventions using evidence-based dressings.
