Qualifications

• Active North Carolina PT license.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility, gait, balance, strength, and safety assessments.
• Develop and implement individualized care plans, including manual therapies, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan, and coordinate with nursing in the multidisciplinary plan as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a touch pad tablet.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents
