Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist for a per diem opportunity in Greenville, North Carolina. The PT will perform home health visits, develop and revise care plans, and collaborate with a multidisciplinary team to help patients remain at home.

Responsibilities include conducting assessments, implementing interventions, educating patients and families, and documenting care in EMR systems. North Carolina PT licensure and an approved PT program are required.

Qualifications

• A current North Carolina Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association–approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; prior EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, balance, strength, and safety.
• Implement wound care and use evidence-based dressings and topical agents.
• Develop and implement individualized care plans with PT interventions.
• Continuously assess and revise care plans in coordination with the nursing team.
• Educate clients and families in rehabilitative care and safety at home.
• Document observations and interventions using electronic records.
