Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for a per diem home health role in the Raleigh area.

The PT will perform home visits in Durham and Wake counties, promoting rehabilitative care for adults and geriatrics and coordinating with a multidisciplinary team. Candidates must hold a North Carolina PT license and have strong problem-solving, communication, and time-management skills; EMR experience is preferred.

Qualifications

• Current North Carolina PT license.
• Graduation from a program approved by the American Physical Therapy Association (APTA).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with electronic medical records (EMR) preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance, gait, etc.).
• Develop and implement individualized care plans including manual therapy and gait training.
• Assess and revise the care plan with the nursing team as needed.
• Educate clients and families on rehabilitative care to promote health and independence.
• Accurately document observations and interventions in the EMR.
