Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Home Health Physical Therapist to perform per-visit home health visits in Forsyth County and surrounding areas in North Carolina.

The PT will join a multidisciplinary team and deliver high-quality rehabilitative care in patients' homes. The role requires current North Carolina PT licensure, completion of an ABA-approved program, and the ability to work independently with strong communication and organizational skills.

Qualifications

• A current North Carolina Physical Therapist license.
• Graduation from a program approved by the APTA or related accrediting body.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance, safety).
• Develop and implement individualized care plans including exercises and gait training.
• Continually assess and revise the PT care plan with the multidisciplinary team.
• Educate clients and family members on rehabilitative care and activities.
• Accurately document observations, interventions, and evaluations in the EMR.
