Job summary

RRC Solutions Group is seeking a dedicated Physical Therapist for a full-time home health role in the Denver area. You will deliver patient-centered care in patients' homes, perform evaluations, and coordinate with physicians to implement effective treatment plans.

Ideal candidates have at least nine months of PT experience, a CAPTE-accredited degree, and an active state license. A valid CPR certification and driver's license with the ability to travel between locations are required.

Qualifications

• Minimum nine months of experience as a physical therapist.
• CAPTE-accredited PT program completion.
• Licensed to practice as a PT in the applicable state.
• CPR certification required.
• Must possess and maintain a valid driver's license and be willing to travel to all patient locations.

Responsibilities

• Provide patient care and document treatment per physician orders and agency policy.
• Perform patient evaluations and select diagnostic materials to assess care.
• Inform physician of assessment results and recommended interventions.
• Develop and document treatment plans with goals and priorities.
• Educate and counsel patients, ensuring organized, efficient treatment.
• Communicate patient status and plans with physicians and team.
