Qualifications

• Minimum nine months experience as a physical therapist.
• CAPTE-accredited degree required.
• Currently licensed to practice as a PT in the operating state.
• CPR certification required.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects diagnostic materials.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans with measurable goals.
• Educates and counsels patients, ensuring organized treatment.
• Shares patient status and treatment plan with physicians and team.
• Provides treatment aligned with patient needs and payer parameters.
• Sets objective, measurable goals with team members and patients.
• Meets productivity standards and uses work hours efficiently.
