Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist to join our Mary Free Bed at Home team in Niles, Michigan. The role is full-time, 40 hours per week, with an 8-hour shift, serving Niles and surrounding areas.

Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state you will work.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to travel ~20% of the day.
• Experience with EMR software and timely charting.

Responsibilities

• Provide home health physical therapy under Mary Free Bed at Home.
• Collaborate with the care team to plan and deliver patient care.
• Manage and deliver patient care plans in the service area.
