Graham Healthcare Group is seeking a Home Health Physical Therapist to support patients across Kalamazoo, MI, and surrounding areas. This role emphasizes delivering patient-centered care within a team-based home health setting and managing treatment plans under Mary Free Bed at Home collaboration.

Qualified candidates have a PT degree, state license, and at least one year of healthcare experience; driving required ~20% of time. Competitive pay per visit and strong benefits package offered.

Formación

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist in the state you will work
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• Commitment to consistently meet critical deadlines for charting
• Demonstrated experience with tablets, mobile devices and EMR software

Responsabilidades

• Provide management and delivery of patient care plans in a home health setting
• Collaborate with Mary Free Bed Rehabilitation Hospital for coordinated patient care
• Support patients in Kalamazoo, Paw Paw and surrounding areas
