Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist in the state you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving.
• Commitment to meeting deadlines for charting.
• Ability to self-manage time and schedule; EMR experience.

Responsibilities

• Manage and deliver patient care plans in home health settings.
• Collaborate with the care team to ensure quality outcomes.
• Adapt therapy plans to patient progress and outcomes.
• Document care activities and tracking in EMR system.
