Graham Healthcare Group seeks a Home Health Physical Therapist in South Haven, Michigan. The role focuses on managing and delivering patient care plans in a home health setting, partnering with Mary Free Bed at Home to ensure high-quality care.

Includes a $10,000 first-year bonus and a competitive pay-per-visit model with generous benefits and schedule flexibility. Must have PT license in the state and 1+ year healthcare experience.

Qualifications

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software

• In partnership with Mary Free Bed Rehabilitation Hospital, Mary Free Bed at Home is an accredited recognized leader in home health.
• This position supports patients in South Haven and surrounding areas and is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year.
• Clinical educators, preceptors, and supervisors to mentor and guide
• A company provided tablet and smart phone with 24/7/365 IT support
