Job summary

Adara Home Health, Inc. seeks a licensed Occupational Therapist to plan and provide skilled therapy in clients' homes, reporting to the Clinical Supervisor. The role requires evaluating, treating, and documenting patient progress with a focus on ADLs and client independence.

You will travel to client homes (1:1 care) within a flexible schedule. Minnesota licensure, AOTA certification, and 2+ years of relevant experience are preferred for employment consideration.

Qualifications

• Licensed to practice Occupational Therapy in Minnesota with no revocation or restrictions.
• Graduate of an AOTA‑approved school and current AOTA certification.
• Minimum of 2 years experience in health care; home care preferred.
• Board certification by NBOT strongly recommended.
• Strong interpersonal, organizational and communication skills.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Administer OT evaluations and develop Plan of Care.
• Set goals, update treatment plans, reassess client needs.
• Provide OT treatments and instructions for ADLs in clients' homes.
• Supervise COTAs, HHAs, LPNs and RNs in therapy programs.
• Document progress, report to physician and Area Clinical Manager.
