Zaddy Solutions in Denver seeks a dedicated Physical Therapist to join our home health team for full-time in-home care. Provide personalized treatment to patients to improve mobility and comfort, aligned with physician orders and agency policy.

Responsibilities include patient evaluations, documenting treatments, developing care plans, and educating patients and families. A CAPTE-approved Master’s, active state license, CPR, and a valid driver’s license are required.

Formación

• Minimum nine months of experience as a physical therapist.
• CAPTE-approved Master’s degree in Physical Therapy.
• Current state license to practice as a physical therapist.
• CPR certification required.

Responsabilidades

• Provides patient care and documents treatment per physician orders and policy.
• Performs patient evaluations and selects diagnostic approaches.
• Develops and documents treatment plans and goals with patients.
• Educates and counsels patients and families; communicates with physicians and team.
