Job summary

ExcelliSearch, Inc. is seeking a dedicated Physical Therapist to join our home health team serving patients in the Denver area.

This full-time role offers the opportunity to deliver personalized care in patients' homes, focusing on improving mobility and quality of life. You will evaluate patients, develop treatment plans, educate families, and coordinate with physicians while meeting productivity standards.

Qualifications

• Minimum of nine months experience as a physical therapist.
• Current licensure to practice in the state of operation.
• CPR certification must be valid while employed in a clinical role.
• Must possess and maintain a valid driver’s license and be able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.
