Job summary

United Future is seeking a dedicated Physical Therapist to join our home health team in Denver. This full-time role involves delivering patient-centered physical therapy in patients' homes, aligning treatment with physician orders, and documenting progress to enhance mobility and comfort.

The ideal candidate has at least nine months of PT experience, preferably in community or home health settings, and holds a CAPTE-accredited Master’s degree.

Qualifications

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects diagnostic materials to assess care.
• Informs physician of assessment results and recommended interventions.
• Develops and documents treatment plans and priorities.
• Educates and counsels patients to support engagement and adherence.
• Presents patient status and treatment plan to physicians and team.
• Delivers appropriate treatment within agency and payer guidelines.
• Sets objective, measurable goals with team and patients.
• Meets productivity standards and uses work hours efficiently.
