The role requires a CAPTE-approved Master’s in Physical Therapy, current state license, CPR certification, and a valid driver’s license for travel to all patient locations. Home health experience is preferred.

Qualifications

• Nine months minimum experience as a physical therapist.
• Preferred community/home health experience.
• CAPTE-approved Master’s in Physical Therapy.

Responsabilités

• Provides patient care and documents treatment per physician orders.
• Performs patient evaluations and informs physicians of results.
• Develops and documents treatment plans and establishes priorities.
• Delivers organized treatment and educates the patient.
• Presents patient information to physicians and keeps team informed.
• Ensures treatment aligns with agency policies and payer guidelines.
• Sets objective, measurable goals in collaboration with patients.
