UnitedHealth Group seeks a Physical Therapist for Guardian Home Health, part of the Optum family. You will assess patient needs, develop and implement a treatment plan, and monitor progress in home settings to improve mobility, endurance, and comfort.

Responsibilities include evaluating functional status, coordinating with the care team, and documenting progress per policy. Required licensure and a valid driver’s license are essential; CPR is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.

Responsabilités

• Evaluates patient function and therapy needs and assists physician in care plan.
• Delivers PT services within license scope and plan of care.
• Monitors patient progress and reports changes to nursing staff/physician.
• Treats to relieve pain, restore function, and maintain optimal functioning.
• Educates patient and family on plan of care and progress.
• Completes evaluations and documents the PT plan of care per state guidelines.
• Reports outcomes and projected frequency of care; works in home settings.
