Job summary

UnitedHealth Group seeks a dedicated Physical Therapist to provide skilled services in home health/hospice contexts. You will assess patients, develop treatment plans, and coordinate with physicians and care teams to improve mobility and function.

Responsibilities include ongoing evaluations, documentation, and collaboration with interdisciplinary staff to optimize patient outcomes. A driver’s license and CPR certification are required for field-based care.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• Graduate of a CAPTE-accredited physical therapy program.
• 1+ year of experience in physical therapy or less than one year with residency/mentorship.
• Current CPR certification.
• 1+ year of experience in clinical assessment, documentation, and care coordination.
• Driver's license and access to reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care with physicians.
• Provide skilled physical therapy interventions and adjust plans based on progress and clinical needs.
• Perform ongoing reassessments to support care planning and coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate progress.
• Maintain accurate, timely documentation in line with regulatory standards.
