Job summary

UnitedHealth Group's Southern Home Care, part of LHC Group within the Optum family, seeks a Physical Therapist for Home Health to assess, plan, and deliver patient-centered care at home. You will evaluate functional status and mobility, develop treatment plans, and work with the care team to improve well-being and outcomes.

Requirements include current PT licensure, reliable transportation, and CPR certification (or within 90 days).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs and assists with care plan development.
• Provides PT services within scope of practice per plan of care and cooperates with the health care team.
• Observes and reports patient responses to treatment and any condition changes.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and care team about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes and anticipated frequency of care; maintains professional standards.
