Job summary

21st Century Home Health Services is seeking a Part Time Physical Therapist Case Manager to provide in-home evaluations and develop care plans for patients in Contra Costa County Richmond area. You will collaborate with physicians, supervise PTAs, and deliver skilled therapy to restore function and independence.

The role requires a CA PT license, BLS/CPR and a CA driver’s license. Strong coordination with the team and community resources is essential for success.

Qualifications

• Current unrestricted CA Physical Therapist License.

Part Time Physical Therapist Case Manager

Coverage Area: Contra Costa County: Richmond

In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment

Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may may also supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. What you’ll do as a Home Health Physical Therapist Case Manager:

• Perform an evaluation in the home and develop a care plan and work with PTA's to follow the care plan.
• Help patients with mobility, movement, and help in regaining their independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a quality level of care.
• Evaluate and report patients’ progress to MD.

To be a success as a Physical Therapist:

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification

• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.

$120,000 - $135,000 a year

Wage range Physical Therapist, Level 1: 120,000.00-135,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match).

As a practice, the majority of PT Case Managers are hired as PTIs. Once it is determined that an PTI can supervise 2 PTAs, take on a larger case load and assume a higher level of decision making and autonomy, the individual will be promoted to an PTII with a minimum salary of 135,000 plus 20K bonus (Paid Quarterly).
