Job summary
Residential Home Health and Hospice is hiring a Physical Therapist (PT) to provide skilled home health care to patients in the Gurnee area. This role offers a total bonus potential of $20,000, including a $10,000 first-year success bonus and a $10,000 sign-on bonus paid within the first 18 months.
You will travel to patient homes within the territory and collaborate with an interdisciplinary team. Qualifications include an active PT license in Illinois and at least 1 year of PT experience, plus
Qualifications
- Active Physical Therapist license in the state you work.
- One year of experience as a Physical Therapist with clinical expertise.
- Driver's license and ability to spend ~20% of your day driving to/from patient locations.
Responsibilities
- Deliver skilled, patient-centered home health care.
- Perform evaluations, mobility assessments, and develop therapy plans.
- Educate patients and caregivers on exercises and safety.
- Complete compliant EMR documentation.
- Travel to patient homes within assigned territory.
