Job summary

Residential Home Health and Hospice is seeking a licensed Physical Therapist to provide skilled home health care in the Plainfield area. The role offers autonomy, collaboration with an interdisciplinary team, and a path for growth, with substantial bonus potential and a first-year success bonus.

The position requires a state PT license, at least one year of experience, and a valid driver's license with the ability to travel to patient homes within the territory.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
