Qualifications

• Active Physical Therapist license in the state you work
• One year of clinical experience as a Physical Therapist
• A valid driver’s license and ability to spend ~20% of workday driving to patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting
• Perform comprehensive patient evaluations and develop therapy plans
• Educate patients and caregivers on exercises and mobility strategies
• Ensure accurate EMR documentation
• Work independently while collaborating with an interdisciplinary team
• Travel to patient homes within assigned territory
