Job summary

Residential Home Health and Hospice in the Gurnee area seeks an Occupational Therapist to provide skilled home health care. You will evaluate ADLs, plan individualized treatments, and work with an interdisciplinary team.

Qualifications

• Active Occupational Therapist license in the state you work.
• Minimum of one year of Occupational Therapy experience delivering patient care.
• Valid driver’s license; ability to travel to patient homes.

Responsibilities

• Evaluate ADLs, upper extremity function, and home safety; develop individualized treatment plans.
• Educate patients and caregivers on adaptive techniques and energy conservation.
