Job summary

Residential Home Health and Hospice is seeking a dedicated Physical Therapist to provide skilled home health services in Glen Ellyn, Addison, Lombard and nearby areas. The role offers up to a $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on paid within 18 months.

Travel required within territory; strong autonomy and patient-centered care are valued. You'll work with an interdisciplinary team, document thoroughly in EMR, and help patients improve

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.
• Valid driver’s license; ability to travel ~20% of workday to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in home health with interdisciplinary team support.
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on exercises, prevention, and safe mobility at home.
• Maintain thorough, compliant EMR documentation.
• Work independently while collaborating with the team and traveling to patient homes within the territory.
