Job summary

VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care in home settings across Texas. You will assess mobility, strength, and balance; develop personalized plans; and collaborate with physicians to maximize independence and safety for patients where they live.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master\u2019s) and licensed to practice in the state
• Current PT license in the state of practice
• Minimum 1- years of home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver\'s license and reliable transportation

Responsibilities

• Deliver patient-centered care to home health patients
• Evaluate and assess functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans with physicians and care team
• Provide hands-on therapy to improve mobility, strength, balance and independence
• Educate patients and caregivers on treatment plans and safety
• Monitor progress and adjust plans as needed
• Support safe transitions of care and discharge planning
• Collaborate with nurses, therapists, and physicians for coordinated care
• Document care and progress in EMR and participate in team meetings
• Supervise PTAs in accordance with state guidelines
